What Are The Options Available With A Breast Augmentation?

Contemporary Plastic Surgery helps patients achieve successful outcomes by effectively defining their aesthetic goals before their procedures. Dr. Herbstman spends extensive time establishing this aesthetic vision for each surgery. In total, patients can expect at least two hours of pre-operative discussions, evaluation, and treatment planning. This allows our practice to consistently meet or exceed patient expectations.

During your initial in-depth consultation, Dr. Herbstman will explain the procedure, answer your questions, and thoroughly review the breast enhancement options that are available, including your breast implant choices. Other topics you may discuss include:

Incision location

To minimize visible scarring after a breast augmentation, Dr. Herbstman will place the incisions in inconspicuous locations such as below the breast within the crease, around the areola, or in the armpit.

Implant material

Cohesive silicone gel offers a remarkably natural look and feel and is the most popular choice for patients who are at least 22 years old. We also offer saline implants, which can be inserted through small incisions and filled after Dr. Herbstman has placed them.

Implant location

The most common placement for breast implants is behind the chest muscles. This approach can be particularly effective when a patient has very little breast tissue before the surgery. Patients can also choose to have their implants placed in front of the chest muscles.

In preparation for their initial consultation, Dr. Herbstman encourages women to bring photos of the type of results they would like to get with breast augmentation. He also advises his patients to review before and after photos on our practice’s website. During the consultation, you and he can discuss these sample cases, and he can help you understand what is achievable given your current breast size and body shape.

By the end of your visit, Dr. Herbstman will recommend the surgical options that are best suited to your anatomy, aesthetic goals, and personal preferences. Patients whose breasts sag significantly might also require a breast lift to get the cosmetic benefits that they want.

I decided for breast augmentation simply because I wanted to look better in my clothes. There was no other reason. I’m not a person that likes to wear low cut tops, so I didn’t do it to be sexier. I just did it because I wanted to look better. I was never ashamed of who I was or how I stood. I just felt I needed that little extra. The first time I went to the doctor, the information he was giving me about the surgery, I was kind of frightened of him. After a few weeks went by, someone gave me the name of the doctor I went to, and when I got to the office, he was such a calm, pleasant man, very caring. Dr. Herbstman’s Experience With Breast Augmentation

Dr. Herbstman has focused on breast surgeries throughout his training and practice as a plastic surgery specialist. He was one of the founding members of the St. Peter’s University Hospital Breast Center and has been providing breast reconstructive procedures to patients since the early 1990s. He has developed and honed his skill over his career, producing cosmetically appealing and natural-looking results.

In addition, Dr. Herbstman is a frequent lecturer and educator on the topics of breast augmentation, lift, reduction, and reconstruction. He has been involved with Susan G. Komen® Foundation for 20 years and is a recipient of their leadership award. In April 2018, he was named president of the New Jersey Plastic Surgery Society, one of the preeminent professional organizations within the American Society of Plastic Surgery.

What Is Recovery From Breast Augmentation Like?

Dr. Herbstman typically performs breast augmentation as an outpatient surgery. Our patients have their procedure in our fully accredited, on-site surgical suite while under twilight anesthesia. The surgery generally lasts one to two hours. Dr. Herbstman uses advanced surgical techniques to reduce recovery time, minimize visible scarring, and provide natural-looking results.

After their breast augmentation, patients can expect some soreness for several days. Dr. Herbstman minimizes post-operative pain by using EXPAREL® during the procedure. This safe and effective medication is the only single-dose treatment proven to control pain for up to 72 hours. As a result, patients can reduce or eliminate the need for opioid medications, catheters, and pumps.

Swelling from breast augmentation may take several weeks to subside completely. In most cases, surgical scars fade considerably within the first few months post-op. As a result of the surgery, patients can expect artfully enhanced, beautiful breasts that are well suited to their body’s frame.

Many of our patients come to their initial consultation unsure about which procedure is right for them. Breast augmentation increases the size of the breasts, but it does not correct sagging as completely as a breast lift. As part of your appointment, Dr. Herbstman will evaluate your needs and make a recommendation about which approach will give you the results you desire. It’s very common for patients to combine breast augmentation and breast lift surgeries to address both volume and shape.

The best location for your incisions will depend on the shape and fullness of your breasts, your body type, your goals with the procedure, and other factors. Each option has its advantages and limitations.

  • The inframammary incision (in the fold beneath the breast) also offers excellent access and avoids the nipple and areola. This approach has less risk of altering sensation to the nipple and is unlikely to interfere with a patient’s ability to breastfeed. Studies have also shown that the inframammary technique poses less risk of scar tissue formation around the implant.
  • The periareolar incision (along the lower border of the areola) gives Dr. Herbstman access for implant placement, particularly when the procedure is combined with breast lift surgery.
  • The transaxillary incision (in the armpit area) leaves no scar on the breast. We generally advise our New Jersey patients that this approach offers less access to placing the implant.

Other factors that are described above, such as the size of your breasts, may affect the incision technique that Dr. Herbstman recommends. No matter which approach you and he choose together, he will make his incisions as inconspicuous as possible to leave scars that heal and fade effectively.

This is a common concern among women who are planning to have children in the future. Getting breast implants should not affect your ability to breastfeed. It is important, however, to communicate your plans to Dr. Herbstman during your consultation. That way, he can plan a procedure with incisions that should not affect your milk ducts.

Dr. Herbstman advises women to follow the guidelines for recovery that Contemporary Plastic Surgery provides to its New Jersey patients. Once patients have recovered fully, they should be able to participate in whatever sports or physical activity they could before they had their surgery.

Technicians who perform mammograms are trained to provide screenings for women with breast implants. You should use a radiology facility where technicians have experience with patients who have had breast augmentation. Make sure to tell the technician you have breast implants during your appointment. The technician may take alternate or additional images of your breasts to ensure a good screening. Note that implants placed under the muscle are less likely to obscure mammograms than those placed above the muscle.

Yes. In addition to breast lift surgery, many of our New Jersey patients pair breast augmentation with body contouring procedures such as liposuction and tummy tuck surgery. When performed together with breast augmentation, these procedures can reshape your entire silhouette. Combining multiple procedures into a single surgery is a safe and efficient way to give patients the results they want, particularly in the hands of a qualified specialist.

You do not have to get your implants changed at any specific time. In over twenty years in practice, Dr. Herbstman has only rarely had patients experience a failure following breast augmentation. While implants cannot be characterized for lifetime use, if they are placed correctly, and patients experience no intervening problems, there is no reason to anticipate that they will require a follow-up surgery to replace them.
